GROOMING RELEASE FORM

^{We do everything possible to make sure your pet’s time with us is as pleasant as possible. Unfortunately, occasionally grooming can expose a previously hidden medical issue, or aggravate a current one. This can happen during or after grooming.}

^{It’s possible that an accident could occur during grooming. Utmost care and caution will always be taken, but cuts, scratches, nicks, and quickening of nails could occur, but The Doggy Barbers cannot be responsible for any conditions or problems discovered during grooming.}

^{The Doggy Barbers will not be responsible for the accidental death of the pet as a result of any pre-existing health condition.}

^{Should we determine, at our discretion, that urgent veterinary care is required, due to a pre-existing health issue, undisclosed or otherwise. I agree to pay all associated fees and costs, for any injury not associated with grooming. If my pet’s vet is not available, I authorise The Doggy Barbers to use it’s chosen vet.}

^{The Doggy Barbers will not be liable for any after-grooming effects of de-matting, clipping, or brushing procedures, or problems uncovered on a badly matted or otherwise neglected coat including, but not limited to, skin redness, itchiness, or self-inflicted irritations from excessive external scratching or rubbing. Time and costs associated with de-matting are unpredictable. I agree to pay whatever fees are incurred as a result of de-matting. Whether a dog is matted is at the sole discretion of The Doggy Barbers.}

^{I agree that I have truthfully informed The Doggy Barbers if my pet has ever bitten another animal or a human, or if they have any other aggressive tendencies. I agree and understand that I will be held solely responsible for any injury, harm, or damage to property caused by my pet.}

^{The Doggy Barbers reserves the right to refuse to groom any pet in an unsatisfactory state, including, but not limited to, behavioural issues, fleas, ticks, other parasites, kennel cough or other contagious diseases.}

^{Our Emmi-pet teeth maintenance service is exactly that – a teeth maintenance service. It is suitable for dogs with none, mild to moderate tartar build up. It can help remove plaque, tartar and freshen breath. The operator is not a veterinarian and teeth maintenance is not a substitute for veterinary treatment where a dog requires dental work beyond cleaning. Owners should be aware of the following:}

^{Removal of even mild tartar can reveal conditions that could not be seen prior to removal and that may need veterinary attention. If the tartar build up on your dog’s teeth is extreme then this will need to be removed by a veterinarian and you may be refused service.}

^{The operator is not a veterinarian and cannot diagnose or treat any oral related problems/illnesses The Emmi-pet toothbrush is a PREVENTATIVE service and is NOT A REPLACEMENT of vet dental check up’s. Depending on how thick and hard the tartar/plaque on your dog’s teeth is, it may take quite a lot of constant sessions to start seeing results. Please note refunds will not be given out once the service has been completed.}

^{Due to no show appointments we also operate a £35 non-refundable booking fee that is to be paid to confirm the first appointment with The Doggy Barbers. We uphold a strict policy on missed appointments and I agree that if I need to change an appointment, I will notify The Doggy Barbers no less than 72 hours before the booked appointment. If insufficient notice is given, I agree to pay 100% of the estimated grooming charges. If appointments are missed with no notice I agree to 100% of the grooming fee and a pre-payment will be required for future appointments.}

^{Please also note that Bank Transfer payments must be paid on the same day as your groom. Late payment fees may apply.}
